About the Position Coordinate patient care from admission to
discharge, develop care plans, collaborate with healthcare teams,
educate patients and families, and ensure safe, efficient, and
cost-effective outcomes. Requirements Requirements: Active RN
license (state or compact) 3–5 years clinical experience (acute
care preferred) Case management knowledge (CCM preferred) Strong
communication and organizational skills EMR/EHR experience Benefits
