AccentCare is seeking a dedicated Weekend Physical Therapist to join our home health care team. In this role, you will be responsible for providing direct physical therapy services, ensuring excellent patient care, and promoting health and independence in a home setting. Your expertise will play a crucial role in creating tailored rehabilitation plans to meet the unique needs of each patient. This position offers the opportunity to work closely with patients and their families, fostering recovery and enhancing quality of life, all while maintaining a healthy work-life balance through a weekend-only schedule. Conduct patient assessments to establish rehabilitation goals and develop treatment plans. Implement physical therapy interventions effectively, based on patient-specific needs. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families on therapeutic exercises and home strategies. Ensure documentation of patient care meets regulatory standards. Collaborate with other healthcare professionals to provide holistic care. Maintain patient confidentiality and respect privacy at all times. Expertise in physical therapy techniques and tools Strong interpersonal skills with an empathetic approach to patient care Proficiency in electronic health record (EHR) systems Ability to adapt treatment plans based on patient response Excellent communication and education ability Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapist license CPR certification A valid driver’s license and reliable transportation, as this role requires travel within the community
