Job Description:

AccentCare is seeking a dedicated Weekend Physical Therapist to join our Home Health team, providing therapeutic techniques for the rehabilitation of patients with various mobility and physical issues at their homes. The role requires travel within the community to implement patient-specific care plans that restore function, alleviate pain, and prevent disabilities. As a home health physical therapist, you will collaborate closely with healthcare professionals to ensure the continuum of care, enabling patients to achieve their full mobility and wellness potential in the comfort of their own homes. Conduct patient assessments and evaluate their physical condition and needs. Formulate personalized care plans to address specific health needs and goals. Implement therapeutic interventions to improve patient mobility and manage pain. Document patient progress and adjust treatment plans as necessary. Educate patients and their families on post-therapy care and exercises. Collaborate with other healthcare professionals to ensure comprehensive care. Proficient in various physical therapy techniques and tools. Ability to assess patient needs and develop effective treatment plans. Strong interpersonal and communication skills. Capability to manage time effectively and make independent decisions. Knowledge of safety guidelines for home health care. Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapy license in the state of practice. CPR or BLS certification preferred.
