visits with coordinated scheduling Highlights: - Provide
comprehensive evaluation and care for home health patients
including initial, resumption, and recertification assessments -
Coordinate multidisciplinary care teams including RNs, LPNs,
therapists, social workers, and home health aides - Develop and
implement individualized care plans in collaboration with
physicians, patients, and families - Deliver age-appropriate
interventions addressing physical, emotional, developmental,
psychosocial, and educational needs - Manage patient episodes from
admission through discharge with therapeutic, preventative, and
rehabilitative focus - Enjoy the autonomy and flexibility of
home-based nursing care Requirements: - Current RN license in
Georgia - Home health nursing experience preferred - Strong
assessment and care coordination skills - Excellent communication
and collaboration abilities across multidisciplinary teams - Valid
